• RSC paperwork completed, signed and turned in to Revolutions Skating Club

• Skater understands to sign in for every session

• Payment is due the 15th of each month

• Any membership package changes must be received in writing prior to the month the day/change will be taking place

• I have read the following: RSC Membership Packet, USFS Skater Code of Conduct, USFS SafeSport Handbook, USFS SafeSport Reporting Process

IMPORTANT: 

Per the rules and regulations provided by US Figure Skating, all the above must be completed before a skater can receive their membership with Revolutions Skating Club.  Skaters may skate one (1) trial session but will not be able to continue to skate unless the above is completed.
